In order to make the day of your evaluation run as smoothly as possible, we have put our intake forms online for you to fill out and turn in prior to your appointment.  This allows our staff to run your insurance benefits, schedule the appropriate amount of time for you evaluation, and minimize the amount of time you spend on filling out these forms in the waiting room.
